Camper Interest FormInformation is vital for our preparation for summer camp. We ask that registered campers and their familiescomplete and return all forms prior to camp. These will also be sent directly to you upon registration.
The medical forms are to be filled out by your child’s physician and by the parents. Campers will not be allowed to stay at camp unless there is a current, signed medical form on file.
